Is it or isn’t it? A new study undermines the theory that sex addiction is a brain-based disorder similar to other addictions. 

For most of the public, the concept of addiction is pretty straightforward — it involves taking something or doing something that brings you pleasure and that you can’t control. But scientifically, addiction means something much more specific, if not precisely quantifiable.

For much of the 20th century, psychiatrists and laypeople defined addictions as use of substances or behaviors that required ever-increasing doses to maintain a satisfying “high.” These addictions also conspired to form some type of physiologic dependence, which led to physical symptoms such as vomiting and diarrhea when the addictive substance or activity was stopped.

But these criteria failed to capture the compulsive quality of addiction, or the craving that drives relapse long after withdrawal symptoms have dissipated. So today’s expert consensus — as laid out in the psychiatric community’s Diagnostic and Statistical Manual of Mental Disorders (DSM) — defines both substance and behavioral addictions, like gambling, as compulsive behaviors that interfere with normal functioning and continue despite negative consequences.

By that definition, excessive sexual behavior also qualifies as an addiction, and a study published last year found that applying these criteria did indeed distinguish people with sex addictions from those with other psychiatric problems. However, the editors of the latest edition of DSM-5 didn’t believe there was enough evidence to support defining hypersexuality as an addiction and decided not to define it as such in the new volume.

At issue is the broader question of what makes behaviors addictive. Since compulsive behavior may have roots in how the brain interprets rewarding or satisfying behaviors, Nicole Prause, a research scientist at the University of California, Los Angeles, and her colleagues decided to start with brain-wave activity to better understand the nature of compulsive sexual behavior.

They studied 39 men and 13 women, all of whom met criteria for hypersexuality, who responded to ads for a study involving people who had trouble controlling their use of pornography. Each of the participants agreed to have their brain activity measured via a noninvasive electroencephalogram, or EEG, while they looked at pictures ranging from pleasant images of couples caressing to sexually explicit scenes, as well as both pleasant and disturbing images of nonsexual activities. Prior to the brain-wave tests, they also completed questionnaires to assess their levels of compulsiveness and control over their sexual impulses.

The researchers focused on brain-wave patterns that occurred about 300 milliseconds after the participants viewed an image, or p300, which measured how interesting or attractive they found that picture. Previous work involving p300 with drug addicts showed that drug-related pictures were far more compelling than other depictions. Finding similar surges in p300 after the participants viewed sexually explicit images would suggest that excessive sexual activity, like some drug use, might be addictive.

Instead, however, Prause and her colleagues found tolerance — unlike drug addicts, the sex addicts in this study didn’t find the sexual cues more compelling than other images. “They look just like normal people with high sex drive,” says Prause. “People who write about sex addiction would say, ‘It’s not just high drive: they are out of control, they can’t stop and their brains are [changed].’ We just don’t see any evidence for them being different.”

That doesn’t mean that excessive or compulsive sexual behavior can’t be seriously problematic. “I don’t think this means that they don’t deserve help or are faking or just being jerks,” she says. But she thinks adding the label addiction could pathologize normal variation and induce pessimism. “I don’t know that we need the overlay of addiction,” she says.

Rory Reid, a research psychologist at UCLA and a colleague of Prause who was not associated with the study, believes the results don’t close the door on the idea of sex addiction or on hypersexual disorder as a diagnosis. Reid was a principal investigator in research that suggested that the criteria for hypersexual disorder qualify it for a psychiatric diagnosis.

He says that the lack of an association between p300 and measures of sexual compulsion isn’t enough to discredit the idea that it’s a brain disorder. “We can’t really assert that,” he says. Just because p300 doesn’t vary in relation to symptoms like loss of control doesn’t mean that other signals, which Prause’s group did not measure, would not. Reid also suggests that Prause’s participants, who were not in rehab programs, may not have been as severely affected as patients in treatment — and that the images used may not have been extreme enough to generate extra interest the way drug images do in addiction.

Why is it so important to determine whether hypersexuality is an addiction? Reid believes it may be more of a research issue than one that has clinical merit, at least for now. “For the patient who comes in after his third job loss because he can’t stop masturbating to porn at work, he doesn’t care what we call it; he just wants to know how to change and function more adaptively,” Reid says.

But determining if common brain pathways are responsible for sexual compulsion and drug addiction could lead to more effective ways of treating sexual desires that start to interfere with daily life. If sexual addiction is similar to cocaine addiction, for instance, the same medications or talk therapies might be useful and the same brain areas could be targeted for treatment.

On Jan. 16, after weeks of sordid allegations regarding the star golfer's extramarital affairs, RadarOnline.com reported that Woods had enrolled in the Gentle Path program at Pine Grove Behavioral Health and Addiction Services, in Hattiesburg, Miss., to be treated for sex addiction. Local television stations later confirmed the story.
Few people know what actually happens at sex rehab. While those who treat it say sex addiction is a disease like any other compulsion, the field is in its infancy: there is virtually no research on it, compared with the vast resources on drug or alcohol addiction. "You look at ways that your behavior has made your life unmanageable. That's really the question," says Benoit Denizet-Lewis, author of America Anonymous: Eight Addicts in Search of a Life, who has been treated for sex addiction himself. "That often differentiates a sex addict from a non-sex addict."

Sex addiction is marked not simply by poor decision-making in the face of temptation, but also by a sense of powerlessness before one's own compulsive behavior. There are many types of sex addicts, including so-called sexual anorexics who avoid physical intimacy with their partner and seek it out in fantasies or with others. Despite the shortage of statistics, researchers agree that the vast majority of sex addicts — over 90% — are men. Rob Weiss, founder and executive director of the Sexual Recovery Institute in Southern California, estimates that up to 5% of Americans deal with some form of sex addiction, though he says there is no real way to know.
Rehab length varies from two-week outpatient seminars to inpatient clinics that keep patients for up to six weeks, like the one where Woods is staying. Treatment — to address both the addiction and its underlying causes — involves a mix of one-on-one sessions, group therapy and family counseling, with addicts and their partners encouraged to participate in supplemental 12-step programs.
The first step in treatment of sexual addiction is a full evaluation of a patient's history and any past trauma. "All the men I've worked with — and I've worked with thousands of them over the years — have some profound experience of abuse and/or neglect in childhood," says Weiss. Without addressing the underlying sexual, physical or emotional trauma that usually leads to addiction, there is little hope of ending it. 

The second stage of treatment involves confronting patients' distorted view of reality. Did the addict really believe that paying for a sensual massage was not the same thing as hiring a prostitute? Or that he or she could spend most of the day surfing the Internet for pornography and that no one would find out? These questions are not meant to shame a patient, but to force him or her to understand what really happened. As Weiss puts it, "We may not stop the behavior, but we're going to ruin it for you."
The last stage of treatment is relapse prevention. Therapists and patients discuss triggers for addictive behavior — unstructured time alone, for example — and identify ways to avoid them. Brian McGinness, a senior cost estimator at a Michigan commercial construction manufacturer, spent the first nine years of his marriage addicted to pornography. His treatment was supervised by members of his church who belonged to an anti-pornography ministry group called XXXchurch and a neighborhood friend, who all acted as "accountability partners," monitoring his Internet usage after he decided to get sober. (Sex addiction shares use of the word sobriety with other forms of addiction, though the definition varies on the basis of an individually determined level of acceptable sexual behavior.) With the monitors' help, which he no longer needs on a regular basis, McGinness has not looked at pornography for the past four years.
A patient's partner can also play an integral role in his or her treatment. Woods' wife Elin Nordegren has already visited him at Pine Grove. "Recovery is a three-legged stool for a couple — his recovery, her recovery and healing, and then the marriage recovery," says Dr. Douglas Weiss (no relation to Rob Weiss), executive director of the Heart to Heart Counseling Center in Colorado, who describes himself as being sober from his own sex addiction for more than 20 years. Addicts are encouraged to disclose the full range of their behavior to their partner when confronting their distortions of reality in the second stage of treatment. If an addict happens to contract an STD and never tells his wife, "his behavior could kill her," Weiss notes. 

Although Woods may be in a six-week program, his therapy is likely to be ongoing. Indeed, at Heart to Heart, clients are encouraged to come back for annual polygraphs to test their sobriety. According to Maureen Canning, a clinical consultant at Meadows Addiction Treatment Center in Arizona, simply working through the addiction could take two to five years of therapy, enhanced by 12-step programs for both partners; working through related trauma could take a lifetime. "Sex addiction is not about remaining abstinent for the rest of your life," says Denizet-Lewis. "It is about learning to have sex in a way that makes you happy again."

A difference between an addict and a recovering addict is that one hides his behavior, while the other can't stop talking about it. Self-revelation is an important part of recovery, but it can lead to awkward moments when you meet a person who identifies as a sex addict.

For instance, within a half-hour of my first meeting Neil Melinkovich, a 59-year-old life coach, sometime writer and former model who has been in Sex Addicts Anonymous for more than 20 years, he told me about the time in 1987 that he made a quick detour from picking up his girlfriend at the Los Angeles airport so he could purchase a service from a prostitute. Afterward, he noticed what he thought was red lipstick on himself. It turned out to be blood from the woman's mouth. He washed in a gas-station bathroom, met his girlfriend at the airport and then, in the grip of his insatiability, had unprotected sex with her as soon as they got home — in the same bed he said he had used to entertain three other women in the days before.
Is this a man with colossally bad judgment or one with a blameless addictive disorder? In the past year, this question has presented itself with dependable regularity. Most famously, Tiger Woods received sex-addiction treatment last winter after he admitted to infidelities; at least a dozen women came forward to claim they'd had sex with him. The chronically undisciplined Charlie Sheen recently sought help in controlling a variety of runaway appetites, including a fondness for the company of porn actresses. Earlier this month, Republican Congressmen Christopher Lee resigned after he was caught e-mailing a shirtless photo of himself to entice a woman he met on Craigslist. And then there is Silvio Berlusconi, the uninhibited Prime Minister of Italy, where prosecutors want him to face trial for accusations that he paid an underage girl to have sex with him. Berlusconi has never hidden his partiality to beautiful women, but he has called the allegations — and reports of louche parties at his villa — politically motivated. All these cases differ in scope, but a central question remains: Why would these men risk everything to satisfy their urges?

When it comes to addiction, the line between morality and disease has always been blurry. But only in the past 25 years have we come to regard excesses in necessary cravings — hunger for food, lust for sex — as possible disease states. In 1983, when Melinkovich was continuously cheating on his then wife (an actress from Planet of the Apes), a Minnesota-based addiction-treatment organization called the Hazelden Foundation published a foundational book called Out of the Shadows: Understanding Sexual Addiction. The book, which is still in publication, helped create the field of sex-addiction treatment. Its author, Patrick Carnes, is now executive director of Gentle Path, the sex-addict program Woods is said to have entered last year in Hattiesburg, Miss.
The American Psychiatric Association (APA) is debating whether sex addiction should be added to its Diagnostic and Statistical Manual of Mental Disorders. The addition of what the APA is calling "hypersexual disorder" would legitimize sex addiction in a way that was unthinkable just a few years ago, when Bill Clinton's philandering was regarded as a moral failing or a joke — but not, in the main, as an illness.

APA recognition of sex addiction would create huge revenue streams in the mental-health business. Some wives who know their husbands are porn enthusiasts would force them into treatment. Some husbands who have serial affairs would start to think of themselves not as rakes but as patients.

This is already happening. In the year since Woods made sex addiction famous, rehab facilities accustomed to dealing with alcoholics and drug addicts have found themselves swamped with requests for sex-addiction treatment. The privately held company Elements Behavioral Health, which operates high-priced rehab centers around the U.S. — including a celebrity-friendly one on a breathtaking mountainside in Malibu, Calif. — recently acquired the Sexual Recovery Institute, a Los Angeles center for sex addicts. The institute's revenues grew 50% in 2010.

But the legitimacy now being granted to sex addiction requires a closer look. In the 20th century, we changed our thinking about alcoholism: what was once a moral weakness came to be understood as an illness resulting in large part from genetics. Why Anthony Weiner Can’t Move Past His Past

It’s not the first time New York mayoral candidate Anthony Weiner has been caught with his pants down — in 2011, he resigned from Congress after he admitted to sending lewd pictures of himself to young women. Yesterday’s revelation that, under the pseudonym “Carlos Danger,” he again sent explicit texts and images to young women more than a year after stepping down from political life raises questions the obvious question— can Weiner’s sexual compulsion be controlled?

Weiner himself has not accepted the label of sex addict, but the candidate’s behavior meets a fundamental criterion for addiction: his exhibitionist acts continued despite negative consequences. It’s hard to imagine a better example of compulsive repetition: although he lost his job and put his marriage, family and entire political future at risk, the former Congressman nonetheless engaged again in the exact type of online behavior that brought him to public humiliation.

Even so, it’s still not clear whether sexual compulsions qualify as an addiction. The latest study argued that they don’t, because hypersexual people process sexual cues just like normal people do— and differently from the way addicts respond to drug cues. But the question is far from resolved.  And whatever you want to call the issue, Weiner still has a problem, since compulsions can be just as disruptive as addictions, and equally difficult to overcome.

Though exact statistics for hypersexual behavior are not known, relapse rates for all compulsive disorders, including gambling and obsessive-compulsive disorder (OCD) are high, and they are generally considered chronic, which means they never really go away, but can re-emerge at any time. 

While he would not comment on Weiner’s case, Rory Reid, research psychologist at the University of California in Los Angeles and a leading expert on hypersexuality, says “Relapse is common in recovery work. Rates appear to vary depending on a number of factors such as the severity or longevity of problem, the presence of co-occurring disorders such as depression, ADHD [attention deficit-hyperactivity disorder] and [others] and an individual’s level of motivation to change.”
Treating sexually deviant behavior also involves two challenges that don’t exist with alcohol and other drug addictions. Sexual behavior is one of the strongest forces in biology, and the need to reproduce represents a fundamental motivation. As a result, moderation, rather than abstinence, is generally the only realistic goal.

Research on substance addictions shows that while moderation is successful in less severe cases, it is often more difficult to achieve. With abstinence, there’s only one option to maintain recovery:  just saying no, every time.  But with moderation, the addict or compulsively behaving person has to figure out when it’s OK to say yes and how to deal with the craving that comes from engaging in just a little bit of the problem behavior without losing control. “When people experience problems with dysregulated behaviors such as eating or sex, it can be much more challenging because the goal is to live in harmony with the ‘substance’ rather than abstain from it,” says Reid. That’s why it’s so hard for people to alter their eating habits, since the desire to eat can’t ever be completely turned off.


And while studies show that the majority of people who suffer from alcohol and substance addictions do recover eventually, usually without formal treatment or even self-help groups, it’s not known if the same is true for sexual compulsions. “There is no published data on long-term recovery rates among hypersexual patients who have received treatment,” Reid says, “This is disconcerting given that rehabs offering treatment for ‘sex addiction’ charge thousands of dollars for their services but have no outcome research to support their claims of being helpful.”

There is also little hope that such research will be forthcoming.  Politicians aren’t eager to fund sex-related research despite the fact that such studies may be the only hope in improving outcomes for not just those with sexual compulsions but for sex offenders as well, who continue to commit sexually related crimes. In the meantime, recovery should be an on-going effort — to be addressed one day at a time — rather than a process with a finite end.
